The piece depicts Madame Sanger, an activist figure from America that pushed the idea of birth control onto those who may need it. The depiction is making her out to be a bad person as she seems like a wicked old woman antagonizing the chinese lady with her three kids. This is most likely a form of propaganda imposing that her ideals were not well liked.
