Amnesiac is the fifth studio album by the English rock band Radiohead, released on 30 May 2001. The album cover was designed by Stanley Donwood (born Dan Rickwood, October 29, 1968), a British artist and writer, and the primary visual artist behind most of Radiohead's works. Donwood collaborated closely with the band’s lead vocalist Thom Yorke to create the visual identity for the band.
The Amnesiac cover features an illustration of a book with a crying Minotaur, which serves as a key symbol within the album’s artwork. The Minotaur represents Yorke’s sense of disorientation and emotional struggle following OK Computer, symbolizing the internal conflict and confusion he faced regarding the band's direction. The cover’s abstract visual style aligns with the album’s complex, experimental sound and introspective lyrics. The special edition of Amnesiac won the Grammy Award for Best Recording Package at the 44th Annual Grammy Awards.
